Wentworth Place Care Service Plans
Living at the Mark Wentworth Home offers you and your family comfort and peace of mind at a great value for the long term. The Service Plans for residents of Wentworth Place have been developed to provide enhanced levels of oversight for activities of daily living, or for medical care as needed. Our Health Services Director will help you determine the right service plan for you during the application process. Here is a brief summary of our Care Service Plans:
A step up from the Basic Services, the Intermediate Plan includes assistance from a staff member with activities of daily living, transferring from sitting to standing, cuing for activities, and/or medication administration by a member of the Home’s licensed care staff.
To prevent our residents from having to move to a nursing home, the Advanced Service Plan provides support from staff for mobility and transfers, nurse intervention for the management of complex medical conditions, incontinence care, and supervision of palliative or hospice services.
Your Care Service Plan is reviewed again after the first six weeks of your residence at the Home, and then again every six months. Additional reviews can be conducted with you and your family at any time.
