A Resident Centered Approach
Garden Place at the Mark Wentworth Home is a special community designed for residents with moderate to advanced cognitive impairments, who require a secured setting and additional attention from staff to meet physical, cognitive and emotional needs. Specially trained staff members support each individual in achieving moments of success, while maintaining dignity and a sense of self worth.
At Garden Place our staff members have been trained to support residents with the changes in mood and physical functioning that come with advanced symptoms of dementia. Our age-in-place philosophy allows residents and their families to receive consistent care in familiar surroundings all the way through palliate or hospice care.
Life at Garden Place
Garden Place offers a carefully designed atmosphere developed to support vision and perception changes that frequently occur in residents with dementia; including secured exits to protect residents who wander or suffer from increased confusion, often referred to as sun-downing.
Meals are served family style, providing an opportunity for residents to see and smell the meal as it is served, which encourages the appetite. Special diets and food preparation methods accommodate residents’ medical and functional needs to ensure proper nutrition.

Advanced Services for Long Term Care
To prevent our residents from having to move to a nursing home, an Advanced Service Plan provides support from staff for mobility and transfers, nurse intervention for the management of complex medical conditions, incontinence care, and supervision of palliative or hospice services.
A Program of Distinction
Garden Place is the first dementia care program in Southern New Hampshire and the only program in the Maine/NH Seacoast area to earn the Excellence in Care Distinction from the Alzheimer’s Foundation of America (AFA). The Excellence in Care program was designed with input from industry experts to serve as a benchmark for dementia care programs across the country.
