To be sure that we are able to provide the level of care specific to each of our residents, we use an assessment tool to determine what their particular need level is.  The following guidelines outline the levels of care available, in addition to the services include in the Assisted Living Core Services.

Care Service Plan Level I

Residents who are looking for the support of a residential care community's ammenities, but may not need the support of a nursing staff for all of their activities of daily living quite yet, are supported with the Level I service plan. 

Our care staff will assist with healthcare management, and provide light assistance transfers or cuing with dressing and grooming, but do not provide support with medication administration.   Residents in this level are safely able to manage their own medications.

Care Service Plan Level II

Typically, residents for whom this plan works best find it difficult to function optimally if living independently. Mobility may be somewhat restricted or recent health issues may lead to a need for periodic health monitoring and assistance with bathing, dressing, grooming or toileting.

Level II includes:

  • medication supervision
  • assistance with health care management
  • complete assistance by one staff member with dressing, grooming, bathing, toileting, or incontinence care
  • assistance by one staff member to transfer from sitting to standing
  • and/or management of mood or behaviors 0-3 days per week

Care Service Plan Level III

The physical needs of a resident may make a higher level of care necessary, while social and intellectual needs are still being met in an assisted living setting. In lieu of a transfer to a nursing care setting, the Level III Care Service Plan makes it possible for a resident to remain active in the assisted living community by adding more care to their core services.

Level III includes services provided in the Level II plan, plus the following:

  • Assistance by two staff members with dressing, grooming, bathing, toileting, or incontinence care, and/or transfers
  • and/or management of mood or behaviors 4 or more days per week
  • A Licensed nurse may assist with:
    • Wound of stage III or greater

    • Management of complex medical conditions

    • Hospice services

    • Coordination of multi-disciplinary health services
